In consideration of permission to use, today and on all future dates, the property, facilities, equipment and services of THE FIELDHOUSE including but not limited to the equipment, fields, facilities, and batting cages, whether owned or leased, for myself, my heirs, personal representatives or assigns, do hereby release, waive, discharge, and covenant not to sue THE FIELDHOUSE, its directors, officers, employees, and agents from liability from any and all claims including the negligence of resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation at such facilities.

I consent to the reproduction, use and distribution of pictures, videos, and sound for the purposes of promotion and advertising to be used in electronic, print, and internet media.
